29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) New residential construction on Lot #72, Front side: On or about 11/03/2017 and at times prior, the employer exposed employees to struck-by hazards, in that protective eyewear was not provided to employees operating circular saws and pneumatic nail guns while engaged in siding installation.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) New residential construction located on Lot #72, Front side: On or about 11/03/2017, and at times prior, the employer exposed employees to fall hazards of 10-feet, 10-inches to the lower level, in that a fall protection system was not provided to employees installing siding on a pump jack scaffold system.   (b) New residential construction located on Lot #72, Back side: On or about 11/03/2017, and at times prior, the employer exposed employees to fall hazards of 13-feet, 9-inches to the lower level, in that a fall protection system was not provided to employees installing siding on a pump jack scaffold system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) New residential construction located on Lot #72, Front side: On or about 11/03/2017, and at times prior, the employer exposed employees to fall hazards of 11-feet to the lower level, in that the ladder used to access the roof was not extended at least 3-feet above the landing or secured to the structure.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) New residential construction located on Lot #72, Roof: On or about 11/03/2017, and at times prior, the employer exposed employees to fall hazards of 11-feet, 1-inch while working from a 5/12 pitched roof, in that a fall protection system was not provided to an employee installing siding using the roof as a walking/working surface.  Bird Home Improvement, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1259456, citation number 1, item number 1 and was affirmed as a final order on 10/27/2017, with respect to a workplace located at 433 Gage Hill Lane, Lexington Park Subdivision (Lot 31) Fairburn, GA 30213.
