Employees changed out blades, and performed cleaning, servicing, and other maintenance on machines such as, but not limited to, two Miehle presses, the cutter, a United M130 folding machine, and the Eastey shrink wrap machine, without utilizing appropriate energy control procedures.  This condition exposed employees servicing this equipment to laceration and amputation hazards.

Employees changed out blades, and performed cleaning, servicing, and other maintenance on machines such as, but not limited to, two Miehle presses, the cutter, a United M130 folding machine, and the Eastey shrink wrap machine, without conducting periodic inspections of the energy control procedures.  This condition exposed employees servicing this equipment to laceration and amputation hazards.    Abatement note:  29 CFR 1910.147(c)(6)(ii) provides that the employer is to certify that periodic inspections have been performed.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    (a) All American Forms Manufacturing, Inc., at 1925 Rosemary St., Unit H, Denver, CO 80231:  On and before November 6, 2017, the employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.  Employees changed out blades, and performed cleaning, servicing, and other maintenance on machines such as, but not limited to, two Miehle presses, the cutter, a United M130 folding machine, and the Eastey shrink wrap machine, without conducting sufficient training of the energy control procedures.  This condition exposed employees servicing this equipment to laceration and amputation hazards.    Abatement note:  29 CFR 1910.147(c)(7)(iv) provides that the employer shall certify that employee training has been accomplished and is being kept up to date. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) All American Forms Manufacturing, Inc. at 1925 Rosemary St., Unit H, Denver, CO:  On and before November 6, 2017, the employer did not ensure that one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards such as those created by the point of operation, ingoing nip points, and rotating parts in the following instances:    1)  Two Ryobi Offset Presses, Model numbers 3302M, included rotating parts and were not adequately guarded at the infeed, top, and outfeed areas;    2)  Two Miehli Presses, used as numbering machines, included rotating parts and were not guarded at the infeed and outfeed areas; and    3)  The United M130 Folding machine included rotating parts and was not guarded at the infeed and outfeed areas.     Conditions exposed employees to laceration and amputation hazards.

The two Miehli Vertical Presses, used as numbering presses, included flywheels near floor level that were not guarded and were located where employees walk and work in the production area. This condition exposed employees to amputation hazards.
