29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On November 6, 2017, located on the south side of a commercial roof structure, employees were observed installing flashing onto the exterior structure as there were no frequent and regular inspections being performed by a competent person to ensure the job-site, materials and equipment such as a fall protection systems and/or devices were being maintained and utilized in a safe condition.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    On November 6, 2017 located on the south side of a steep pitch commercial roof, employees were exposed to a fall hazard while installing metal flashing to commercial exterior structure wearing their harness that was not attached to the rope and anchor system. Potential fall height measured to be 20-7. Roof pitch was measured to be 5/12.
