29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about October 31, 2017, employees of Rockford Ball Screw Company, Inc., performed servicing and maintenance tasks when performing setups on the following machines: a. Vertical Mill, Mazak V-414, Serial Number 106982 (V 414) b. Lathe, Mazak QTN 30, Serial Number 125950 (Lathe 6) c. Lathe, Index C 200, Serial Number 720511 (C200A) d. Lathe, Index C 200, Serial Number 720706 (C200B) e. Lathe, Mazak SQT 300 MY, Serial Number 164146 (Lathe 8) f. Lathe, Mazak SQT 18 MY, Serial Number 120961 (Lathe 0) g. Lathe Mazak QTN 200 MY, Serial Number 184312 (Lathe 20) h. Lathe Mazak QT18N Univ, Serial Number 84891 (Lathe 1) Employees were relying on safeguarding in lieu of lockout/tagout when performing these tasks. The safeguarding was not adequate alternative protection to be used in lieu of lockout/tagout and did not meet ANSI B11.19-2010 requirements for control reliability. Employees were exposed to amputation and laceration hazards from the unexpected start up and/or the release of hazardous energy. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The employees were exposed to amputation and laceration hazards. This item was corrected during inspection.
