29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Jobsite: On November 2, 2017 and at times prior thereto employees were installing plastic on a sheathed roof. No fall protection system was used. Fall hazards ranged from 23.3 feet to over 25 feet. Note) Certification of corrective action is required for this item.

29 CFR 1926.502(b)(13): When guardrail systems are used around holes which are used as points of access (such as ladderways), they were not provided with a gate, or be so offset that a person cannot walk directly into the hole: a) Jobsite: On November 2, 2017 and at times prior thereto employees used an extension ladder through a floor opening for access to an attic area. This area was then used for access to the roof hatchway. A guardrail system was not in place around the floor opening. Employees were exposed to a 9 foot fall hazard. Note) Certification of corrective action is required for this item.
