The employer failed to report the work-related in-patient hospitalizations to OSHA within twenty-four (24) hours:    a) Employee 1 was admitted to the Freeman Hospital West (FHW) on November 3, 2017 at approximately 10:56 am due to a work-related incident that occurred the morning of November 3, 2017. This in-hospitalization report was approximately 52 hours and 23 minutes late.    b) Employee 3 was admitted to the Freeman Hospital West (FHW) on November 3, 2017 at approximately 10:54 am due to a work-related incident that occurred the morning of November 3, 2017. This in-hospitalization report was approximately 52 hours and 25 minutes late.    c) Employee 2 was admitted to the Freeman Hospital West (FHW) on November 3, 2017 at approximately 10:58 am due to a work-related incident that occurred the morning of November 3, 2017. This in-hospitalization report was approximately 52 hours and 21 minutes late.     d) Employee 4 was admitted to the Freeman Hospital West (FHW) on November 5, 2017 at approximately 1:57 pm due to work-related a incident that occurred the morning of November 3, 2017.
