Employees were exposed to fall hazards during residential construction activities while doing rip and tear roofing work with no fall protection systems in place. Occurred on 11/01/17.

Employees were exposed to fall hazards while using a damaged aluminum extension ladder to access the roof of a three story building. Occurred on 11/01/17.
