On or about November 7, 2017, employees working on a 4:12 pitch roof were not utilizing fall protection. Employees were exposed to a 10 ft. fall to ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite at 2740 W. 12th St. in Erie, PA. On or about November 7, 2017, employees were not trained to recognize fall hazards or procedures to follow to minimize those hazards. Employees were not utilizing fall protection when exposed to a 10 ft. fall to ground while working on a low-sloped roof.
