29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: (a) Freezer Packing Warehouse - The employer did not prepare a certification of hazard assessment which prescribes the personal protective equipment necessary to protect the food production workers from the identified hazards, on or about November 6, 2017. (b) Maintenance - The employer did not prepare a certification of hazard assessment which prescribes the personal protective equipment necessary to protect the maintenance workers from the identified hazards, on or about November 6, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
