29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) On or about 01/11/18 in the Printing department; employees made adjustments to the anilox, changed/cleaned the doctor blades and seals, on the pressman and assistant side of Press 6-1 and were exposed to an unprotected fall of 54 inches while they stood on a work surface 3.5 inches wide by 55 inches in length. b) On or about 01/11/18 in the Press department; employees made adjustments to the anilox, changed/cleaned the doctor blades and seals on the pressman and assistant side of Press 6-2 and were exposed to an unprotected fall of 57 inches while they stood on a work surface 3.5 inches wide by 55 inches in length. c) On or about 01/11/18 in the Press department; employees made adjustments to the anilox, changed/cleaned the doctor blades and seals on the pressman and assistant side of Press M-8 and were exposed to an unprotected fall of 51 & 84 inches while they stood on a work surface 3.5 inches wide by 60 inches in length. d) On or about 01/11/18 in the Press department; employees made adjustments to the anilox, changed/cleaned the doctor blades and seals on the pressman and assistant side of Press F&F and were exposed to an unprotected fall of 84 inches while they stood on a work surface approximately 20 inches wide by 60 inches in length. e) On or about 01/11/18 in the Press department; employees made adjustments to the anilox, changed/cleaned the doctor blade and seals on the north side of newly installed Bobst press and were exposed to an unprotected fall of approximately 84 inches while they stood on a work surface approximately 24 inches wide by 60 inches in length.
