29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):    On or about October 31, 2018, an employee suffered multiple finger amputations while performing servicing and maintenance work on the Ellis Corporation Washer Extractor #2 (Model #Z472J) located in the washing machine area.  The employee was adjusting the drive belt without performing the necessary lockout steps outlined in paragraph (d)(1) through (d)(6).
