The employees were installing metal roofing materials on a new commercial building low slope roof with no fall protection system/device.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about November 7, 2017, at a worksite located at 5085 Park Avenue West, Seville, OH 44273 during commercial roofing work, employees were exposed to fall hazards of approximately 18 feet when the employer did not ensure fall protection was worn by employees. Elite Ceiling Systems LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1257372, citation number 01, item number 002 and was affirmed as a final order on September 12, 2017 , with respect to a workplace located at 1348 South Cleveland Massillon, Copley, OH 44321
