29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On or about November 8, 2017, an employees was exposed to a fall of approximately 14-16 feet. The employee was installing siding materials on a new residential home located 2074 Spring Ridge Drive, Twinsburg, Ohio 44087. An employee was working from a ladder jack scaffold without using any form of fall protection.
