On or before November 3, 2017 where a 13 foot long section of 4 inch ductile iron pipe was rigged to be lowered into a lift station, the pipe was not rigged to prevent accidental displacement which resulted in the pipe slipping out of the rigging and striking an employee, resulting in the hospitalization of the injured employee.

The employer must train each competent person and each qualified person regarding the requirements of this subpart applicable to their respective roles. a. On or before November 3, 2017 where a 13 foot long section of 4 inch ductile iron pipe was rigged to be lowered into a lift station, the employer failed to adequately train the employees on correct rigging procedures, resulting in the pipe slipping out of the rigging and striking an employee. The resulting injuries required the hospitalization of the injured employee.
