29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:     On or about November 8, 2017, during the installation of waste water line in the 11000 block of Royalton Rd, North. Royalton Ohio 44133, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to cave in hazards where employees were working in a trench with varying depths of 8 to 9.7 feet.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).     On or about November 8, 2017, at a job-site located at 11000 block of Royalton Rd, North Royalton Ohio 44133; the employer did not ensure adequate cave-in protection was used to protect an employee working inside a trench with near vertical side walls and a depth varying from approximately 8 to 9.7 feet.  The trench was determined to have type "B" soil.
