29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees: The employer is failing to protect workers from the hazards of falls and striking objects in the work area when working from a scaffold. This violation was most recently documented at a worksite located at 4444 West Pine Boulevard, St. Louis, Missouri, where workers were allowed to put a plank between the midrails of the scissors lift guardrail system to create a temporary platform to permit workers to reach areas they could not while standing on the floor of the lift.

This violation was most recently observed at a worksite located at 4444 West Pine Boulevard, St. Louis, Missouri, where workers were required and/or permitted to work from a platform supported on the midrail to gain access to work locations at the third level balcony which placed the employees above the provided fall protection.
