29 CFR 1910.95(b)(1): 29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized. If such controls fail to reduce sound levels within the levels of Table G-16, personal protective equipment shall be provided and used to reduce sound levels within the levels of the table. Employees operating printing presses were exposed to noise hazards in that the sound levels were in excess of those in Table G-16 in that feasible administrative and engineering controls have not been utilized. This was most recently observed on November 16, 2017 at 500 SW Gary Ormsby Dr. in the National Print Center.

29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: Employees engaged in printing operations in areas with TWA noise exposures over 85dB were exposed to hearing loss in that employees were not required to use greater hearing protection when employees sustained a standard threshold shift.
