29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a. On or about November 8, 2017, two employees sealing leaks around an unprotected skylight were not protected from falling 14 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about November 8, 2017, employees used a 20- foot extension ladder to access and exit from the roof.  The ladder was not extended at least 3 feet above the upper landing and was not secured to prevent accidental displacement.
