29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not netralized, power was not shut off, and brakes set. a) Jobsite-On or about November 7, 2017, an unattended Sunbelt Telehandler Rough Terrain Forklift (Model: JLG 6042 forklift S/N 879266) with the boom elevated was left unattended while the forks were being loaded with scaffold components, exposing employees working on the scaffold to falls and crush-by hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) Jobsite-On or about November 7, 2017 employees were working from the second tier of a tubular welded frame scaffold with exposed open sides and ends in that the employer did not ensure fall protection was provided such as, but not limited to, guardrails, exposing employees to fall hazards up to 14 feet.
