Worksite located at 819 E. 31st St., Kansas City, Missouri- Employee was observed working on a ladder-jack scaffold and was not protected from falling by the use of a guardrail system or personal fall arrest system.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    (a). Worksite located at 819 E. 31st St., Kansas City, Missouri- Employee was working on a ladder-jack scaffold and had not received any training on the safe usage and measures to protect himself from falling to the lower level.
