29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) On or about 11/7/2017 - 800 Block of Indian Creek Road, Huntsville, AL, the employer failed to ensure that employees working in a trench excavation 8-feet deep, 3-feet wide, and 200-feet long without cave-in protection were instructed to avoid the trench/excavation hazards.
