Cross braces were used as a means of access. a. On or about November 8, 2017, at the above addressed jobsite, employees conducting roofing operations from a ladder jack scaffold, eighteen (18) feet four (4) inches above the ground, were not provided with an access ladder, thereby exposing the employees to falls.
