29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.    a.  On or about November 8, 2017, at the above addressed jobsite, employees conducting roofing operations from a ladder jack scaffold, eighteen (18) feet four (4) inches above the ground, were not provided with an access ladder, thereby exposing the employees to falls.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a.  On or about November 8, 2017, at the above addressed jobsite, employees performing roofing operations from a ladder jack scaffold, eightteen (18) feet four (4) inches above the ground, did not utilize a personal fall arrest system, thereby exposing the employees to falls.

On or about November 8, 2017, at the above addressed jobsite, employees performing residential roofing operations, eightteen (18) feet four (4) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.

On or about November 8, 2017, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
