29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails were not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On 10/4/2017, employees of Byron Friedhoff, Inc. were framing the roof of a residential building located at 405 Mason Creek Drive, Lake Saint Louis, MO. The ladder the workers were using to access the roof was not extended three feet beyond the upper landing surface of the roof.

29 CFR 1926.501(b)(13): "Residential construction." Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems. On 10/4/2017, employees of Byron Friedhoff Inc. were working on a residential building located at 405 Mason Creek Drive, Lake Saint Louis, MO. Employees were engaged in framing work on a gable approximately 10 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below. Byron Friedhoff, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1004216, citation number 1, item number 1 and was affirmed as a final order on 11/21/2014, with respect to a workplace located at14 Hills Drive, Glendale, MO.
