29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. LOCATION: Ames Production Department, Ragozzino Foods, Inc. 10 Ames Avenue, Meriden, CT 06451: On/or about November 09, 2017, the employer did not ensure through visual observation, appropriate communications and other means that the hazardous energy control (lockout/tagout) procedures were utilized during the sanitation operations (tasks) on equipment and machinery, such as (but not limited to) the press mixer/augur. On/or about November 09, 2017, an employee of Packers Sanitation Services, Inc. (PSSI) sustained finger amputation and/or laceration while performing the above tasks on the above equipment and machinery whose power source was not shut-off and locked out by utilizing appropriated procedures and devices.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. LOCATION: Ames Production Department, Ragozzino Foods, Inc. 10 Ames Avenue, Meriden, CT 06451: On/or about November 09, 2017, the employer did not conduct lockout/tagout training to ensure that hazardous energy control (lockout/tagout) procedures were utilized during the sanitation operations on equipment and machinery, such as (but not limited to) the press mixer/augur exposing an employee who was performing the above tasks to caught-by and caused crushing and amputation injuries.
