LOCATION:  Ames Production Department, Ragozzino Foods, Inc. 10 Ames Avenue, Meriden, CT 06451:      On/or about November 09, 2017, the employer did not ensure through visual observation, appropriate communications and other means that the hazardous energy control (lockout/tagout) procedures were utilized during the sanitation operations (tasks) on equipment and machinery, such as (but not limited to) the press mixer/augur.    On/or about November 09, 2017, an employee of Packers Sanitation Services, Inc. (PSSI) sustained finger amputation and/or laceration while performing the above tasks on the above equipment and machinery whose power source was not shut-off and locked out by utilizing appropriated procedures and devices.
