29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Facility Wide, Mansir Printing, 24 Shawmut Avenue, Holyoke, Massachusetts; On and before November 14, 2017 employees use a variety of printing equipment and chemicals in the facility including but not limited to Q6516 coating which listed a level B personal protective equipment requirement on the safety data sheet.  The employer had not performed a hazard analysis to determine what personal protective equipment was required by employees performing production tasks.

The shrink wrap machine was operated with the guard in the open position. b.  The outlet rollers on the MOB machine were not guarded to keep operator's hands from nip points. c.   There were chains on an MOB machine and the Yagenberg Single Line Folder machine that were not guarded. d.  The chain on the first Kluse machine was not guarded and employees were exposed amputation hazards from rotating parts and nip points.

In the printing room, extension cords powering equipment including microwaves and personal electronics.
