29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  a)  Second buck of scaffold at front-face of structure and outrigger located on left face of structure;  On or about November 7, 2017 and at times prior thereto, the Controlling employer, Danny Clements, Builder, Inc., failed to ensure that it's subcontractor, Walter Smith Brick Company, protected its employees from fall hazards of up to 16 feet 6 inches to the ground/concrete below while employees were working from the second buck and outrigger of a scaffold that was erected without all the required base plates.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  a)  Scaffold at front-face of structure;  On or about November 7, 2017 and at times prior thereto, the Controlling employer, Danny Clements, Builder, Inc., failed to ensure that it's subcontractor, Walter Smith Brick Company, protected its employees from fall hazards of up to 15 feet 3 inches to the ground below while employees were climbing up the side supports of a scaffold to access the 2nd buck and outrigger without the use of a ladder or other means of safe access.
