29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    On or about November 14, 2017, the employer did not provide employees adequate training to ensure that they understood the purpose and function of hazardous energy control program and that they acquired the skills necessary for safe application of energy control for servicing Bag Line machines, including at the post gusseter roller section of machine 14. Employees were exposed to fractures, lacerations and amputation hazards while webbing.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).

29 CFR 1910.147(d): Application of control. The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    Bag Lines - On or about November 14, 2017, the employer failed to ensure that employees were protected against unexpected start-up while servicing production machinery including, Bag Line Machine 14. Employee(s) performing webbing tasks at the post gusseter roller, had their fingers exposed to caught-in hazards. The following hazardous energy control (lockout) steps were not performed to protect the employee(s):    (1) prepare for shut down by recognizing energy hazards and its controls,  (2) turn off and shut down the machines,  (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4) affix lockout or tag-out devices to each energy isolating device,  (5) render safe all potentially hazardous stored or residual energy,  (6) prior to starting work, verify that isolation and de-energization of the machine or equipment have been accomplished.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Bag Lines - On or about November 14, 2017, the employer failed to develop and document energy control procedures for employees to follow while servicing production machinery including, Bag Line Machine 14. Employees were exposed to nip point hazards, likely to result in amputation while servicing the bag line.     Colonial Bag Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1044883, citation number 1, item number 1 and was affirmed as a final order on 09/30/2015, with respect to a workplace located at 205 Fullerton Ave. Carol Stream, IL. 60188.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
