29 CFR 1910.178(q)(6):   Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, and/or altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section. Additional counterweighting of fork trucks was not approved by the truck manufacturer.    On or about 11/13/2017 at 225 WE Green Jr. Parkway, Byron, GA:  Employees operated powered industrial trucks with an metal attachment connected to the forks of the lift that was used to lift and move light and heavy recreational vehicles. The employer altered the powered industrial trucks without providing the employees a manufacturer approved attachment.  The employer indicated that they have two (2) non-approved attachments that they use to lift and move the equipment.

On or about 11/13/2017, in the maintenance and service shop area at 225 WE Green Jr. Parkway, Byron, GA: Employees are exposed to an explosive hazard where oxygen, acetylene (flammable), and argon compressed cylinder were being stored together.
