29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  a. On or about November 14, 2017, and at times prior thereto, employees were performing service and maintenance covered activities in the robot cells and inside the laser enclosures, such as, but not limited to, cleaning, adjustments, replacing nozzles, and replacing lenses. The employer did not develop and implement specific energy control procedures for authorized employees to utilize to isolate and control each hazardous energy source, such as, but not limited to, lockout point locations, lockout devices to operate, and removal of residual and/or stored energy.

29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).   a. On or about November 14, 2017, and at times prior thereto, employees were performing service and maintenance covered activities in the robot cells on Lasers 3 & 4 and inside the Laser 1 through 8, laser enclosures, such as, but not limited to, cleaning, adjustments, replacing nozzles, and replacing lenses. The employer did not ensure that employees isolated and affixed a lockout device to the hazardous energy sources prior to permitting them to perform the covered activities in order to ensure each employee performing the work had control over their own safety. The safety interlock plugs remained hanging outside the guarded areas while employees performed the work.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.   a. On or about November 14, 2017, employees were operating a Toyota, serial number 10328, propane powered forklift with a hydraulic leak. The condition of the forklift exposed employees to a crushed by hazard.
