29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface had side rails that extended at least 3 feet above the upper landing surface. On or about 14 November 2017, and at times prior thereto, employees accessed the rooftop of an apartment building to replace an HVAC unit with an extension ladder that was not extended at least 3 feet above the upper landing, exposing the employees to a fall hazard of approximately 25 feet to the ground below.

On or about 14 November 2017, and at times prior thereto, employees were replacing an HVAC unit on the rooftop of an apartment building, while employees on the ground were lowering or raising tools and equipment by ropes without head protection, exposing the employees on the ground to the hazard of being struck by falling tools, materials, and debris.
