29 CFR 1926.452(k)(2): All ladders used to support ladder jack scaffolds did not meet the requirements of subpart X of this part as required:  a)  Ladder Jack Scaffold - Employees worked and walked on a ladder jack scaffold platform supported by two portable extension ladders where the ladders were set up at an unsafe angle, on or about November 14, 2017. The horizontal distance from the top support to the foot of the ladders exceeded one-fourth of the working length. The foot of the ladders measured between 9 feet and 10 feet from their top supports and should have measured no more than 5 feet.   No abatement certification or documentation required.

Employees used a TransSporter ladder hoist for access and egress.  No abatement certification or documentation required.
