29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on November 14, 2017 on the jobsite located at 2818 Eagle Hills Cir, Papillion, NE 68133.   One employee was performing a roof inspection on the roof of a 2-story duplex while construction work was in progress.  The roof has a 6 in 12 steep sloped pitch at heights greater than 6 feet above the ground.        The controlling contractor is failing to ensure that all employees of the sub-contractor Isaias Torres are protected from fall hazards while working on a residential roof.  This most recently occurred on November 14, 2017 on the jobsite located at 2818 Eagle Hills Cir, Papillion, NE 68133.  Eight employees from Isaias Torres were observed working on the roof of a 2-story duplex where there is a 6 in 12 steep sloped pitch at heights greater than 6 feet above the ground.      Aspen Contracting, Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number  1187640, citation number 1, item number 1, and was affirmed as a final order on March 1, 2017 with respect to a workplace located at 5741 Whiskey River Dr, Colorado Springs, CO 80922.     Aspen Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1013162, citation number 1, item number 1, and was affirmed as a final order on February 10, 2015, with respect to a workplace located at 10243 E. Peakview, Centennial, CO 80111.    Aspen Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 944176, citation number 1, item number 2a, and was affirmed as a final order on October 10, 2014, with respect to a workplace located at 8380 N. Booth, Kansas City, MO 64158.
