29 CFR 1910.37(b) (4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent: a. 250 Industrial Park Circle, Lawrenceville, GA (Throughout Facility) - Employees were exposed to entrapment hazard. There were several designated, marked, and identified emergency exit doors located throughout the facility which were hidden by numerous large stacks of warehouse products on tiers of storage racks, which blocks the view of the exit routes, signs and doors. There were no directional exit markings indicating the way to travel, on or about 11/15/17.

29 CFR 1910.178(l) (4) (ii) (A): Refresher training in relevant topics was not provided to the operator that has been observed to operate the vehicle in an unsafe manner: a. 250 Industrial Park Circle, Lawrenceville, GA (Shop Area) - Employees were exposed to struck by hazard. Employees were not provided refresher training after being involved in forklift incidents, near misses and other unsafe acts. The employees were required to continue to operate the forklift without being retrained in the relevant topic, on or about 11/15/17.
