On or about November 3, 2017, at the entrance of the directional drilling rig walkway, the means of egress from the directional drilling rig was not kept free of obstruction at all times when the excavator superstructure rotated towards a trailer to unload drill pipe. Employees were exposed to hazards including, but not limited to falling drilling pipe or fires. b. On or about November 8, 2017, at the walkway to the drillers cabin, the means of egress was obstructed by hoses. Employees were exposed to hazards including, but not limited to fires.

On or about November 8, 2017, directional drilling rig, the first section of the rig walkway had a hole and exposed employees to trip hazards. b. On or about November 8, 2017, at the earth walkway to the drillers cabin, an area of the fence ran over the mud pit creating a hole into which employees could step and exposed employees to trip hazards.
