29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) On or about 11/29/2017 throughout establishment; where energy control procedures (Lockout/Tagout, LOTO) were required for equipment such as, but not limited to: the DeLaurice press brake (model no. 22510, serial no. 31696), the Hercules shear (serial no. 500120117522) and the horizontal band saw, the documented LOTO program and procedures were not reviewed to identify deficiencies (at least) annually. ABATEMENT CERTIFICATION REQUIRED
