29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On  December 12, 2017 Truman Gingerich, dba Crown Coatings and Industrial Services, was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:                 a.  Citation 1 Item 1, and         b.  Citation 2 Item 1.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.     On or about November 14, 2017, Employees performing roofing activities were exposed to a fall hazard on a commercial construction site in Bement, Illinois, and fall protection was not being used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about November 14, 2017, Employees were exposed to a fall hazard, while accessing and exiting the work area via an extension ladder on a commercial construction site in Bement, Illinois, and the employer did not ensure the extension ladder extended at least 3 feet above the upper landing surface.    Truman Gingerich dba Crown Coatings and Industrial Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1158295 citation number 1, item number 2 and was affirmed as a final order on September 27, 2016, with respect to a workplace located at Super 8, 207 S. Murray Rd., Rantoul, IL 61866.
