29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about November 15, 2017, at the above addressed site, an employee was exposed to a fall hazard of approximately 13 feet when performing stucco preparation work from the basket of an aerial lift while not wearing a harness with a lanyard attached to the basket or boom of that lift.

29 CFR 1926.453(b)(2)(ix): Controls for the articulating and extensible boom platforms were not clearly marked to identify their function:  On or about November 15, 2017, at the above addressed site, an employee was exposed to a fall hazard when performing stucco preparation work from the basket of an aerial lift while the lift's controls were covered with stucco material and not plainly marked as to their function.

29 CFR 1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):   On or about November 15, 2017, at the above addressed site, employees were using chemicals including but not limited to Lanco Non-Rewettable Concrete bonding Agent & Additive CB-1000, Cemex Broco Stucco Cement and Gasoline while the employer did not maintain the safety data sheets for that products at the worksite.
