29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) Mafco Worldwide Corp. - Warehouses: The Toyota Forklift #38, Model 405FGU25, Serial 70370 had damaged or malfunctioning parts such as damaged tail lights, headlights, etc. Forklifts that had been taken out of service as a result of maintenance needs were not secured to ensure that they were not utilized until the necessary maintenance or repairs were performed, observed on or about 09/15/2017.
