Overhead door #9 - On or about November 10, 2017, an employee received a serious amputation injury while performing maintenance on the Liftmaster (model MT5011U S/N 4614N9029) overhead door opener, where the power source was not physically located, isolated and deenergized prior to beginning the maintenance procedures.
