29 CFR 1926.25(a):  During the course of construction debris such as; scaffolding components, plastic conduit, pallets, boards, insulation, bricks and blocks, was not kept cleared from work areas and passageways around the building.    JOBSITE:  Employees were exposed to the hazard of tripping while walking/working in close proximity to debris, such as (but not limited to) broken pieces of concrete and stone that were scattered in and around the walking/working areas.

JOBSITE:  A ground fault circuit interrupter (GFCI) or an assured equipment grounding program was not used where employees were using a portable power tool which was powered by an electrical extension cord.
