29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection. a) Third Floor, Building 4 - On or about November 14, 2017, the employer did not ensure that anchorages were adequately installed for personal fall arrest systems while employees were framing a three-story apartment building, thus exposing employees to fall hazards up to approximately 21 feet, 4 inches.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) Jobsite - On or about November 14, 2017 and times prior to, employees were not instructed in the correct procedures for erecting personal fall arrest systems, thus exposing employees to fall hazards up to about 21 feet, 4 inches.
