On or about 11/15/17 - Facility's laboratory, personal protective equipment such as face shield and gown or jacket were not provided and utilized where employees had occupational exposure to blood and other potentially infectious materials during cleaning and disinfection of re-useable instruments. b. On or about 11/15/17 - Facility's laboratory, safety eye protection was not provided and used during the processing of lab specimens.
