Building 7301, Maintenance room; On or about 15 November 2014 and times prior thereto; a Performax, 6-inch Bench Grinder, model 90250, serial number WP071152; did not have the work rest closed adjusted to within 1/8 inch of the abrasive wheel. The work rest was 1/4 inch from the abrasive wheel. This condition exposed employees to a caught in hazard.

Warehouse Building 800, an air nozzle used by employees tested at over 100 p.s.i.          b. Moose Creek Golf Course, Grass Blade Conditioner Building, an air nozzle used by employees tested at approximately 65 p.s.i.  This condition exposed employees to air embolism/laceration hazards.

Building 794, bay 2: On or about 14 November and time prior thereto, an energized electrical outlet was contaminated with paint. The outlet was filled with paint. This condition exposed employees to a fire hazard.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage.  673 Force Support Squadron Facilities; On or about 15 November 2017 and times prior thereto; the employer did not ensure that the electrical panel boards were readily accessible. Seven instances were noted:        Instance a. Outdoor Recreation Facilities Building 794, adjacent to the equipment issue counter; an electrical panel board had miscellaneous items stored in front of the panel impeding immediate access.       Instance b. Outdoor Recreation Facilities Building 7301, adjacent to the Outdoor Adventure Program, an electrical panel board had a chair stored in front of the panel impeding immediate access.       Instance c. Outdoor Recreation Facilities Building 7301, adjacent to the mezzanine stairs trash cans and water bottles were stored in front of the three electrical panels impeding immediate access.       Instance d. Hillberg Ski Area Building 23392, adjacent to south entry doorway; electrical panel board labeled FXSB 009108 had several miscellaneous items were stored in front of the panel impeding immediate access.        Instance e. Quartermaster Laundry Building 726, dry cleaning area, northwest wall: electrical panel board labeled FXSB 000871 has a laundry cart was stored in front of the panel impeding immediate access.        Instance f. Polar Express Tool Room Issue area; on the left side of tool issue counter adjacent to east entry door. A trash can was stored in front of the electrical panel labeled FXSB016381 impeding access.       Instance g. Right side of tool issue counter adjacent to west entry door. A table was stored in front of the electrical panel labeled FXSB016383.  This condition exposed employees to an electrical shock hazard.

Outdoor Recreation Facilities Building 794, boat bay 1, center stanchion;  a knockout plug was missing on the left hand side of an energized electrical outlet box.        Instance b. Outdoor Recreation Facilities Building 794, boat bay 5, a knockout plug was missing from the left hand side on an energized electrical box.       Instance c. Warehouse Building 800, Northwest Wall, a 120 volt electrical junction box was missing a knock-out on the top left hand side.         This condition exposed employees to an electrical shock and electrical burn hazard.

Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface.  Outdoor Recreation facilities:  On or about 15 November 2017 and times prior thereto; three instances were noted where electrical openings were not effectively closed:      Instance a.  Building 794, boat bay 1, two electrical openings on an energized electrical box were not effectively closed.  Two of the rocker switches were covered with duct tape.      Instance b.  Building 794, welding area, adjacent to work bench; two electrical openings on an energized electrical outlet box were not effectively closed.  Two of the rocker switches were covered in duct tape.      Instance c.  Building 794, adjacent to welding area entrance; and electrical opening on an energized electrical box was not effectively closed.  These condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Outdoor Recreation and Polar Express facilities; on or about 14 November 2017 and times prior thereto; the employer did not ensure that electrical cable on portable equipment, was free of damage. Three instances were noted:       Instance a. Outdoor Recreation, Building 794, Bay 5; a 50-foot black heavy duty energized 220-power cord had damage to the outer protective jacket adjacent to the plug.        Instance b. Outdoor Recreation, Building 7301, Maintenance room; a 100-foot yellow extension cord was damaged on the outer protective jacket adjacent to the receptacle plug.        Instance c. Polar Express, Building 755, Woodworking area; a Porter Cable dustless belt sander, model 360 HD, serial number 068321, power cable was free of damage. Power cord had damage to the outer protective jacket.   This condition exposed employees to an electrical shock hazard.
