The employer is failing to protect employees from fall hazards associated with performing residential construction activity while exposed to falls 6 feet or greater.  The most recent occurrence of this was observed Nov 15, 2017 at the existing residential home located at 4963 S. 176th Ave, Omaha, NE where the employer has 2 out of their crew of 5 employees exposed to fall hazards while tearing off the roof of an existing residential home with roof pitches exceed 6:12.

The most recent occurrence of this was observed Nov 15, 2017 at the existing residential home located at 4963 S. 176th Ave, Omaha, NE where the employer has a crew of 5 employees using an aluminum ladder that was not extended a minimum of 3 feet over the eve of the home.
