29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. Production shop, CNC Mazak horizontal milling machine no. 1, model no. FH-4800, serial no. 145603 - on or about November 16, 2017, employees operating equipment were exposed to metal chips and coolant struck-by hazards along with other moving components caught-in hazards because access door's safety interlock was by-passed, and other protective measures were not ensured when accessing danger zone (machining area) to perform production related work tasks. There is no abatement certification or documentation required for this item.
