29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Roof - On or about November 15, 2017, employees installing roof sheathing on a two-story residence were exposed to fall hazards up to approximately 19 feet, 8 inches onto earthen and concrete ground because the workers were not fall protected. b) Second Floor - On or about November 15, 2017, an employee working/walking at a second-story wall was not fall protected, thus exposing the employee to fall hazards up to approximately 12 feet, 5 inches onto earthen and concrete ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite - On or about November 15, 2017 and times prior to, employees were not instructed in the hazards associated with their work duties including but not limited to fall protection requirements while performing residential construction work, thus exposing employees to fall hazards up to about 19 feet, 8 inches onto earthen and concrete ground.
