29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite - On or about November 15, 2017 and times prior to, employees were not instructed in the hazards associated with their work duties including but not limited to fall protection requirements while performing residential construction work, thus exposing employees to fall hazards up to about 12 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Jobsite - On or about November 15, 2017, employees used an aluminum extension ladder to gain access to the roof of a one-story residence without the side rails of the ladder being extended at least 3 feet above the upper landing surface, thus exposing employees to fall hazards of up to approximately 10 feet onto earthen ground.
