29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of 29 CFR 1910 was not worn and attached to the boom or basket when working from an aerial lift: On or about November 16, 2017, at the above addressed jobsite, an employee was observed servicing a sign, from an aerial lift, wherein the employee was not wearing a personal fall arrest system, nor wearing a travel restraint system that met the requirements in subpart I of this part, therefore exposing the employee to a fall hazard. NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS CITATION.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment: On or about November 16, 2017, at the above addressed jobsite, an employee was performing maintenance/repairs on sign from the basket of an aerial lift, during which the employer did not provide training to each employee who is required by this section to use PPE, and wherein the employee was required to use PPE/PFAS, therefore exposing the employee to a fall hazard and/or suspension trauma/orthostatic shock hazard. NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS CITATION.
