OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hand and finger injuries by inadvertently or accidentally stepping on the exposed foot pedal while positioning the rolling rod on vinyl rolling machine #1 to the correct position: (a) Rolling Machine #1: On or about 11/15/2017, the foot pedal on vinyl rolling machine #1 was not covered to prevent unintended activation of the machine, exposing employees to hand and finger injuries while positioning the rolling rod.
