29 CFR 1910.178(l)(3)(i)(G): Fork and attachment adaptation, operation, and use limitations. a. On or before November 9, 2017 where an employee was operating a forklift to empty a hopper, the employee was not trained on the safe operation of the hopper, resulting in the fingertip amputation of the employee's ring finger of the left hand. b. On or before November 16, 2017 where employees operated forklifts to empty hoppers, employees were not trained on the safe operation of the hopper, thereby being exposed to crushed-by hazards.
